DEGREE OF XEROSIS AMONG ELDERLY PATIENTS WITH AND WITHOUT DIABETES MELLITUS AT GOTONG ROYONG HOSPITAL SURABAYA

Abstract

Background: Xerosis is common in older adults due to impaired skin barrier function, reduced stratum corneum hydration, and increased transepidermal water loss (TEWL). The condition may be more severe in individuals with uncontrolled diabetes due to mechanisms such as peripheral neuropathy, anhidrosis, accumulation of advanced glycation end products, and reduced skin surface lipids, leading to further skin dehydration. Xerosis in diabetic patients may lead to pruritus, an increased risk of infection, and decreased quality of life. Objective: To analyze differences in xerosis severity between older adults with uncontrolled diabetes and those without diabetes mellitus. Methods: This cross-sectional study included 66 older adults, comprising 33 with uncontrolled diabetes mellitus and 33 without diabetes mellitus, selected by consecutive sampling. Xerosis severity was assessed using the Overall Dry Skin (ODS) score. Statistical analysis was performed using the Mann-Whitney test, with p <0.05 considered statistically significant. Results: Xerosis severity was significantly higher in older adults with diabetes mellitus (p <0.001). Most diabetic participants had grade 3 xerosis (51.52%), whereas most non-diabetic participants had grade 1 xerosis (54.55%). Conclusion: Xerosis severity is significantly higher in older adults with uncontrolled diabetes mellitus, indicating the importance of early identification and appropriate skin management in this population.
