Background: Acute Respiratory Tract Infection (ARI) is a major cause of infant morbidity and mortality in Indonesia. Despite 80% exclusive breastfeeding coverage in Surabaya, the incidence of ARI in children remains high. Recurrent ARI is defined as 3 or more episodes of ARI within 6 months, which can impact growth disorders, reduced quality of life, and increased risk of complications. Objective: To analyze the effect of exclusive breastfeeding on recurrent ISPA in children aged 9–24 months at the Gotong Royong Clinic in Surabaya. Methods: An analytical observational study with a case-control design. A sample of 54 children (27 cases of recurrent ARI and 27 controls of non-recurrent ARI) was selected using a consecutive sampling technique. Exclusive breastfeeding and ARI frequency were measured using a questionnaire to determine whether the child was breastfed or not and how many times the child experienced ARI within six months. The data results were processed and analyzed using the chi-square test using SPPS to see whether there is an effect between exclusive breastfeeding and recurrent ARI. Results: A total of 24 children (44.4%) received exclusive breastfeeding, and 30 children (55.6%) did not. Children who received exclusive breastfeeding were 16 children (66.7%) who did not experience recurrent ARI and 8 children (33.3%) who experienced recurrent ARI. Children who did not receive exclusive breastfeeding were 11 children (36.7%) who did not experience recurrent ARI and 19 children (63.3%) who experienced recurrent ARI. There was an association between exclusive breastfeeding on recurrent ARI in children aged 9–24 months at the Gotong Royong Clinic, Surabaya with a p value = 0.028 (p <0.05). Conclusion: There is a significant influence between exclusive breastfeeding and recurrent ISPA in children aged 9–24 months at the Gotong Royong Clinic in Surabaya.
